ACL Rehabilitation in London: The Elite-Sport Approach

By Geoff Scott, Founder & Senior Physiotherapist, Hauora

ACL reconstruction takes about an hour in theatre. The rehabilitation takes nine months or more, and it’s the rehab, not the surgery, that decides whether you get back to the level you left. In professional football, that process is run as carefully as the operation itself: daily measurement, clear criteria and no guesswork. At Hauora in Primrose Hill, we bring the same model to every patient, whether you play in the Premier League or on a Sunday morning.

Why the elite-sport approach matters

Geoff Scott spent 20 years leading medical teams at Tottenham Hotspur and Nottingham Forest. He has guided professional footballers through ACL reconstruction and back to top-flight competition. That experience shapes every Hauora programme: you move to the next stage by hitting objective targets, not by counting weeks.

1. Restoring full knee extension

In the first weeks after surgery, our top priority is getting the knee fully straight. Losing extension, even by a few degrees, is one of the most common and costly complications after ACL reconstruction. A knee that won’t straighten changes how you walk, overloads the front of the knee and, critically, stops the quadriceps working properly.

We aim to match the extension of your other knee, including any natural hyperextension, as early as it’s safe to do so. We get there with targeted manual therapy, positional work, gait retraining and a clear home programme that we review at every session. Bending (flexion) matters too, and we restore it progressively, but extension comes first.

2. Switching the quadriceps back on

After surgery, the nervous system partly switches off the quadriceps. This protective reflex is called arthrogenic muscle inhibition. It’s why many patients can’t do a straight-leg raise or lock the knee in the first days. Extension and quad activation go hand in hand: you can’t fully contract the quad on a bent knee, and a weak quad struggles to hold the knee straight.

We use early isometric work, neuromuscular electrical stimulation and, where appropriate, blood flow restriction training to restore activation while protecting the graft.

3. Building strength that holds up under load

Quadriceps weakness is the biggest single barrier to a successful return to sport, and it can last for years if it isn’t addressed properly. Your strength programme starts with bodyweight work and builds to heavy, carefully dosed loading of the quads, hamstrings, glutes and calves. We introduce open- and closed-chain exercises at the right time for your graft type.

We measure rather than estimate. We test your strength regularly against your uninjured leg, and we want at least 90% symmetry before you move on to high-level running, cutting and jumping.

4. Proprioception and movement quality

Strength alone doesn’t protect a knee. The ACL contains sensory receptors that help your brain know where the knee is, and reconstruction disrupts that feedback. We retrain it step by step: single-leg balance and perturbation work, then landing mechanics, then hopping, deceleration and change of direction that match the demands of your sport.

We look closely at how you land and cut. The movement patterns behind the first injury often remain after surgery unless they’re deliberately retrained.

5. Returning to sport on criteria, not the calendar

Research shows that returning to sport within nine months of ACL reconstruction significantly increases the risk of reinjury, and that risk falls with each month you wait up to that point. At Hauora, the return-to-play decision is based on data: strength symmetry, a battery of hop tests, movement quality, sport-specific conditioning and psychological readiness. It’s the same framework used in professional football, and it means you know your knee is ready, not just that enough time has passed.
