If you need hamstring repair rehab in London, you’re far from alone. Sport has never been faster or more explosive, and hamstrings are paying the price.

Why hamstring ruptures are on the rise

The UEFA Elite Club Injury Study has tracked Europe’s top football clubs since 2001. Over that time, hamstring injuries have doubled from 12% to 24% of all injuries. Between 2006 and 2013 alone, Premier League sprint distance rose by about 35% and the number of sprints per match by about 85%. Most hamstring injuries happen while sprinting, as the muscle works at full stretch to slow the leg down.

It isn’t only professionals. Amateur footballers, rugby players and active adults in their 40s and 50s are also suffering severe tears and proximal hamstring avulsions that need surgical repair. At Hauora, physio after hamstring surgery is one of the most common programmes we run.

Hamstring surgery rehab with the Hauora team

Our hamstring repair rehab is led by founder Geoff Scott, Head of Medicine and Performance at Tottenham Hotspur for 20 years, and Jason Oliveira, Senior Physio at Saracens Rugby Club. They’re supported by our wider team of medical, recovery and performance specialists, who bring elite-sport methods to every patient.

With us, you get:

  • A clear plan with measurable goals, agreed with your surgeon and starting soon after your operation
  • Objective testing of your strength, range of movement and sprint speed against your uninjured leg
  • Advanced facilities at At The Core
  • One team from start to finish, from the protected early weeks to full-speed sprinting
  • A long-term plan to prevent reinjury after you’re discharged

The 5 stages of hamstring repair rehabilitation

1. Protect the repair. For roughly the first six weeks, the tendon is healing back onto the bone. We follow your surgeon’s protocol, which may include a brace and crutches, and avoid the position that stresses the repair most: hip bent with the knee straight. Meanwhile we manage pain and swelling, start gentle muscle activation, and monitor the sciatic nerve, which runs right beside the repair.

2. Restore mobility without overstretching. Aggressive stretching can put a repair at risk. Instead, we restore range through controlled movement and loading, treat scar tissue and nerve mobility, and track your hip flexion against your other leg.

3. Rebuild hamstring strength in the right order. You start with isometrics, then move on to hip-focused work such as bridges and Romanian deadlifts. Next come knee-focused exercises, building up to the Nordic hamstring exercise, which has been shown to roughly halve hamstring injury rates. Finally, we train the muscle to absorb force as it lengthens. Before full return, we want no more than a 10% strength difference between your legs.

4. Run, then sprint. Hamstring rehab can’t finish in the gym. We bring running back on the AlterG, then move outdoors through tempo running, accelerations and progressively faster sprints until you can reach close to top speed repeatedly, without symptoms.

5. Return to sport on criteria, not the calendar. The decision is based on your strength, range, sprint speed, performance in sport-specific drills and your own confidence. You leave with a maintenance programme, because a previous hamstring injury is the biggest risk factor for another. Read more in our guide to returning to play after injury.

Hamstring repair recovery time: FAQs

How long is recovery after hamstring repair surgery?
Most patients are off crutches by around six weeks and running again from around three months. Return to sport typically comes four to six months after a proximal hamstring repair, depending on the injury and your surgeon’s protocol.

When should I start physio after hamstring surgery?
Usually within the first week or two, working within your surgeon’s guidelines.
